Health Forms

Key's online health management webpages, called Magnus Health, are available in July once new families are given access to the Parent Portal. In the meantime, downloadable health forms that require a physician's or parent's signature are available below now to give new families time to schedule doctor's appointments. The forms below must be signed by a physician and/or parent and returned to Key School by August 9, 2019.

Forms may be scanned and emailed to School Nurse Katie Anderson at kanderson@keyschool.org or mailed to the Key School Health Office, 534 Hillsmere Drive, Annapolis, MD 21403.
