Magnus Health
Magnus Health, Key's secure online health management site, is available to newly enrolled families starting in mid-July after new families are given access to the Family Portal.
In order to get a head start on forms that require a physician's signature, we have made them available below to download. These forms will also be available in the Family Portal in July, however, we realize setting up doctor's appointments can take time, so they are available to you now.
The forms below must be signed by a physician and/or parent and returned to Key School by August 12, 2024.
Forms may be scanned and emailed to School Nurse Katie Anderson at kanderson@keyschool.org or mailed to the Key School Health Office, 534 Hillsmere Drive, Annapolis, MD 21403.
If you have questions or wish to discuss specifics regarding your child's health, please contact Nurse Katie directly.
Immunization Policy
- All students must be fully immunized according to the Maryland State requirements for school attendance; the School will not accept any religious exemptions.
- All Health Forms must be completed and returned to Key's Health Office by August 12.
What is Magnus Health?
The secure Magnus Health site is where Key families provide important online medical consents and emergency contacts. Key's Health Office collects and stores all student health and medical information through Magnus Health, the leading provider of web-based student medical record software.
